More Than a Provider.
A Partner in Independent Living.

Palmer's Home Care LLC has spent more than two decades equipping, educating, and empowering individuals with developmental and intellectual disabilities to live full, meaningful lives across Mid-Missouri. From our first residential home to more than 50 across the region, everything we do is built on one belief: people with disabilities deserve the same rights, opportunities, and quality of life as their neighbors — and the right support makes that possible.

Who We Are & Why We Exist

Our Mission

To equip, educate, and empower individuals with disabilities to live unique, healthy, and fulfilling lives in their homes and communities.

Our Vision

We believe people with disabilities are no different from anyone else. They share the same hopes, dreams, desires, and frustrations as their neighbors — and they deserve the same human rights. At Palmer's Home Care, we walk alongside individuals as they build independence, pursue meaningful relationships, and grow in self-reliance.

Palmer's Home Care team

Our Story

Palmer's Home Care was founded in Columbia, Missouri with a simple conviction: people with developmental disabilities deserve to live in real homes, in real neighborhoods, surrounded by people who genuinely care.

Over the past 20+ years, that conviction has grown into one of Mid-Missouri's most trusted DD service providers. Today, Palmer's Home Care operates more than 50 residential homes, 7–8 community day program sites, and a regional training hub — serving individuals and families across Boone County, Randolph County, Cole County, and surrounding communities.

We've never chased growth for its own sake. Every home we've opened, every staff member we've hired, and every program we've launched has been guided by one question: Does this help the people we serve live better lives?

The Way We Work

Our approach to support isn't institutional — it's intentional, personal, and built on genuine relationships.

Real Homes. Real Neighborhoods.

We don't operate group facilities or institutional settings. Every individual we serve lives in a real home, in a real neighborhood — capped at two residents per home, so every person has space, privacy, and genuine community connections.

The Peer Model

Our staff aren't authority figures — they're partners. We hire people who see the individuals they support as equals, and we train them to teach and model rather than direct and control. The result is relationships built on trust, not compliance.

Community Integration

Meaningful life doesn't happen inside four walls. Our day programs, employment services, and residential support are all designed to connect individuals with their communities — real jobs, real friendships, real participation in the places they call home.

Palmer's Home Care & Mid-Missouri Supported Living

Palmer's Home Care LLC (PHC) and Mid-Missouri Supported Living (MMSL) are two separate organizations united by shared leadership and a shared mission. Both operate under the guidance of Heath Clark and Gary [Last Name], coordinating to serve a broader range of individuals and needs across the region.

Palmer's Home Care LLC (PHC)

Provides residential support services, day programs, employment services, and staff training across Mid-Missouri — guided by a person-centered approach that puts individual goals and community participation first.

Mid-Missouri Supported Living (MMSL)

While PHC and MMSL function independently, their coordination allows them to serve a broader range of individuals and needs — with the same commitment to quality, community integration, and person-centered support that defines both organizations.
